Connect Oregon is an effort to connect health care, human, and social services partners to improve the health and well-being of communities. OHLC and technology partner Unite Us launched Connect Oregon in October 2020.
Health care and social service providers come together through Connect Oregon to take a more coordinated approach to address the social health of individuals they serve. Partners work together, on a common technology platform, to send and receive electronic closed-loop referrals, address people’s social needs, and improve health across communities.
Connect Oregon is sponsored by numerous partners led by Samaritan Health Services and InterCommunity Health Network CCO (IHN-CCO), Kaiser Permanente, Central Oregon Health Council, PacificSource, CareOregon, Columbia Pacific CCO, Jackson Care Connect, Health Share of Oregon, AllCare Health, Eastern Oregon CCO, Yamhill Community Care, Umpqua Health Alliance, and Trillium Community Health Plan. Connect Oregon operational partners include 211info as a coordination center, social service organizations as network referral partners, and an Oregon-based Unite Us community engagement and support team. Oregon Health Leadership Council is a statewide convener for the network.
