
Connect Oregon is an effort to connect health care, human, and social services partners to improve the health and well-being of communities. OHLC and technology partner Unite Us launched Connect Oregon in October 2020.
CONNECT OREGON
Health care and social service providers come together through Connect Oregon to take a more coordinated approach to address the social health of individuals they serve. Partners work together, on a common technology platform, to send and receive electronic closed-loop referrals, address people’s social needs, and improve health across communities.
Connect Oregon is sponsored by numerous CCO and health system partners and the ODHS Office of Resilience and Emergency Management (OREM). Connect Oregon operational partners include 211info as a coordination center, social service organizations as network referral partners, and an Oregon-based Unite Us community engagement and support team. Oregon Health Leadership Council is a statewide convener for the network. Conversations continue with state agencies and other health care partners to help fund this statewide effort.
